Obezite Cerrahisi nedir

Vücut Kitle İndeksi 40 ya da üstünde olan kişiler ya da Vücut Kitle İndeksi 35 – 40 arasında olmakla beraber obeziteye bağlı yandaş hastalıkları bulunan hastalara cerrahi tedavi uygulanabilmektedir. Ülkemizde Sosyal Güvenlik Kurumu (SGK), Vücut Kitle İndeksi değeri 40’ın üstünde olan hastalara cerrahi tedavi onayı vermektedir. Kurumuzda da obezite cerrahisi SGK anlaşmalı olarak gerçekleştirilmektedir.
 
Obezite Cerrahisi Çeşitleri Nelerdir?
Cerrahi yöntemler mantıksal olarak “Gıda alımını kısıtlayıcı”, “Besin emilimini azaltıcı” ya da bunların birleşimi biçiminde bir sonucu amaçlamaktadır.
 
Burada başarılı olan ve yaygın uygulanmakta olan 3 cerrahi yöntemden söz edilecektir:
 
Ayarlanabilir Mide Bandı ( Lapband ) : Halk arasında mide kelepçesi olarak da bilinir. Laparoskopik bir operasyonla midenin etrafına silikondan bir band yerleştirilmesi işlemidir. Bu aparat, cildaltına yerleştirilmiş bir hazne yolu ile sıvı enjekte edilerek şişirildiğinde mideyi kum saati biçiminde sıkıştırmakta ve gıda alımını sınırlamaktadır. Hastanın durumuna göre sıkılık ayarlanabilmektedir. Başarılı olguların yanında, yabancı cisim etkisiyle mide dokusunun rahatsız olmasına bağlı komplikasyonlar da gözlenebilmektedir.
 
Tüp Mide (Sleeve Gastrektomi): Laparoskopik bir operasyonla, midenin yemek borusu ve barsakla sürekliliğini sağlayacak tüp biçiminde bir parçası korunarak, mide gövdesinin büyük bölümünün çıkarılması işlemidir. Midenin kapasitesinden ve iştahı yöneten hormon salınımından sorumlu bölümleri çıkarılmış olduğundan gıda alımı sınırlanmaktadır. Normal barsak yolu korunduğu için daha fizyolojik bir yöntem olup, besin emilim bozukluğuna bağlı komplikasyonlar oluşmamaktadır. 1 – 2 senede verilmesi gerekli olan kilogramnun %70 – 80’i verilebilmektedir.
